Solicitation Requirements

In addition to requirements expressly listed in the solicitation, the following items are necessary when responding to a solicitation:

• Completed and Signed W-9

• Business Information Questionnaire Form

• Insurance

This list does not include all possible insurance requirements or endorsements that may be requested. The coverage and limits vary based on the scope of work.
